Center of Excellence: The Southeast Partnership for Health
The Southeast Partnership for Health Center Of Excellence (COE) is a Ryan White funded program focusing on providing services to residents of the Bay View Hunter’s Point neighborhood of San Francisco. The COE is comprised of a partnership that includes Southeast Health Center, Bay View Hunter’s Point Foundation, Black Coalition on AIDS, UCSF Positive Health Practice Men of Color Program, and Maxine Hall Health Center.
The overarching goal of the COE is to ensure a high quality, accessible, comprehensive culturally competent, and fully integrated HIV services program that extends the quality and length of life for low-income HIV infected and affected Southeast residents.
SERVICES
COE services include:
- Community outreach
- Health care evaluation and assessment
- Care coordination
- Referral and linkage to primary care, mental health, and substance abuse services
STAFF
The COE staff includes:
- Program Coordinator
- Clinical Care Coordinators
- Care Coordinator
- Program Assistant
REQUIREMENTS
To receive treatment, a client must be:
- 18 years or older
- Resident of the southeast corridor of San Francisco
- HIV+ diagnosis
